Sculptra
A collagen biostimulator, not a filler. Slow, regenerative volume that builds from inside the skin over months.

What it is
The clinical, plainly stated.
Sculptra is poly-L-lactic acid (PLLA). The particles are read by your fibroblasts as a scaffold, and your skin lays down new collagen around them over weeks and months. The volume that shows up is yours.
It is the opposite of a quick fix. The trade-off is patience for results that look more like skin and less like a deposit.
Protocol
The series, and where it goes.
Sculptra is a series, not a one-time treatment. Below is the protocol I use most often, plus the regions I treat.
The protocol
- 01
Two vials per session is standard for the face. Body areas often need more.
- 02
Two to three sessions, spaced four to six weeks apart, is the typical build.
- 03
Maintenance lands every 18 to 24 months for most clients.
- 04
Dilution and reconstitution matter, under-dilution and poor massage are where nodule risk comes from. I dilute generously and massage in-clinic before you leave.
Areas treated
Temples
Sculptra's most rewarding area, slow, structural fill in a region that's hard to overdo when done with PLLA.
Cheeks and midface
Subtle, long-arc collagen build that supports cheek architecture over time.
Preauricular
Skin quality and structural support in front of the ear, often paired with temples.
Jawline
Soft, supportive build along the lower face, quiet definition that compounds with each session.
Body, off-label
Buttocks, thighs (crepey skin), arms, and abdomen. Body treatments need more vials and a longer series.
Ideal candidate
Who I treat, and who I send back.
Mild-to-moderate volume loss with early laxity.
Wants gradual, subtle change, and doesn't need to see results in a mirror tomorrow.
Willing to commit to a series and to the at-home massage discipline (the 5-5-5 rule below).
Not pregnant or breastfeeding, no autoimmune connective tissue flares.
What to expect
The visit, start to finish.
- 01
Consult + plan
We map the regions, decide vial count, and review the at-home massage protocol before anything goes in.
- 02
Reconstitution
Sculptra arrives lyophilized. I reconstitute with sterile water plus lidocaine, generously diluted. This step quietly determines safety.
- 03
Cleanse + numb
Topical or a small amount of injectable lidocaine for tender areas.
- 04
Inject
Threading and fan techniques into deep dermis or subcutaneous plane, depending on the region. Cannula is common for safety in mid-face.
- 05
In-clinic massage
We massage the area thoroughly before you leave. This is where even distribution starts. You'll look plumper from the diluent water, that fades in 24 to 72 hours.
Results timeline
When you'll actually see it.
Day 0–3
Plumper-from-water look. False fullness from the diluent, it will fade.
Week 2–4
Fluid absorbs, collagen response begins. You may look like you did before the appointment. This is expected.
Week 6–12
Collagen ramps. Subtle structural changes start showing in photos.
Month 3–6
Peak visible improvement. This is the window I judge results against.
Up to 2 years
Duration of the collagen build. Maintenance lands somewhere in here.
Aftercare
The first 24–48 hours.
Boring instructions that quietly determine how good your result looks at week two. Don’t skim them.
- 01
The 5-5-5 rule: massage the treated area for 5 minutes, 5 times a day, for 5 days. Clean hands, with lotion.
- 02
Avoid extreme heat, sauna, hot yoga, for 24 to 48 hours.
- 03
No facials, microneedling, or laser for two weeks.
- 04
Cool compress and elevated-head sleep for the first night if face was treated.
- 05
If you feel a lump, don't poke at it. Call the clinic, we have protocols and they work.
FAQ
The questions I get most.
When will I see results?
Some fullness from the diluent shows up in the first few days, then fades. True results are visible at four to eight weeks. Peak is three to six months. Patience is the price of admission.
How long does it last?
Around two years on average, sometimes longer. Maintenance every 18 to 24 months is common.
How is Sculptra different from filler?
Filler is gel, it adds volume directly, hydrates, contours, and is reversible if it's HA. Sculptra is a scaffold, your own collagen builds around it over months. Not reversible, but it resorbs over time. Different tools, often used together.
How many sessions will I need?
Two to three sessions, four to six weeks apart, two vials per session is the face standard. Body needs more.
Is it safe?
It has a strong safety record when diluted and placed correctly and when you do the massage. Nodule risk is real if technique or aftercare is sloppy, which is why I'm strict about both.
Can I combine it with other treatments?
Yes. Sculptra works well alongside neurotoxin, HA filler in targeted areas (like lips or chin), and tightening devices. We plan the order intentionally.
Honest risks & contraindications
What can go wrong, and where we say no.
Sculptra has a high ceiling and a low floor, when technique and aftercare are right, the result compounds quietly over months; when they're not, the complications can take months to resolve. Here's what we screen for.
Common + expected
- ·Redness, swelling, and bruising at injection sites.
- ·Brief asymmetry as the early water plumping fades unevenly.
Serious, uncommon but real
- ·Nodules, technique- and aftercare-dependent. Can take months to fully resolve. The 5-5-5 massage rule is non-negotiable for this reason.
- ·Granulomas, rare, treatable.
- ·Infection, rare.
We don’t treat if
- ·Pregnancy or breastfeeding.
- ·Active skin infection in the area.
- ·Allergy to PLLA or lidocaine.
- ·Severe keloid history, case by case, often a no.
- ·Uncontrolled autoimmune or connective tissue disease.
- ·Active anticoagulation, bruising risk gets discussed openly first.

Filler is usually the last tool I reach for, not the first. When we do use it, we use a little, in the right plane, to support what your own skin and collagen are already doing.
